Description

Act as a lead specialist on safeguarding and provide support, advice and guidance in the treatment and/or management of others, when dealing with complex conditions and/or situations, adhering to national guidance and local policy.

Work in partnership with the other services/stakeholder partnership agencies to enable patients/clients to be given appropriate treatment in the appropriate setting.

Undertake complex investigations in relation to safeguarding making recommendations as required.

Develop partnerships and joint working with service users and stakeholders to improve patient/client care; this will include attendance at the Safeguarding Adults Board subgroups.

Implement plans for the team including rotas and schedules / working patterns to ensure business continuity.
Organise and plan own lead area work streams.

Work in partnership with other organisations, including statutory organisations to support the effective and co-ordinated provision of health and social care services.

Communicate at all levels of the organisation to a variety of health and social care professionals to provide best outcomes for patients/clients.

Provide support and advice to those staff managing complex and vulnerable adults on their caseloads.
Challenge professional and organisational boundaries to ensure that the role is focused on meeting the needs of service users.
Act as a role model to others ensuring that patients receive the most effective care possible.
Practice autonomously and demonstrate evidence based clinical decision making.
Provide clinical expertise and knowledge to others when managing complex and highly complex situations. Where required refer to other specialists to contribute to ensure the most appropriate management of a service user.

Assess patient conditions / situations and consider a range of options when advising in relation to complex and highly complex care, drawing on specialist services as required.
