We are currently moving from a traditional model of general practice to a more multi-disciplinary model and we are keen to recruit one or more Advanced Care Practitioners to lead this change.


The ACP(s) will play a key role in moulding the current GP, nurse, paramedic and pharmacy team into a new model of clinical care that reflects the needs of the practice and establishes a cutting edge approach to primary care.

A strong, well established management/administration team stands ready to support this change.


Main duties of the job
You will be responsible for comprehensive health assessments for adults and children and will be capable of working autonomously.

We would be delighted to hear from non-medical prescribers who are willing to consider a new ACP-led model of primary care.


Job responsibilities
The post holder will be an Advanced Clinical Practitioner (ACP) (All Non-medical prescribing professionals will be considered)


The post holder will:
- act as an autonomous practitioner for the surgery with involvement in the implementation and monitoring of all clinical aspects of patient care
- provide day-to-day clinical care


Key Responsibilities
A. Practice Finance

  • Support on QOF Areas, working with the practice staff, ensuring that we reach our QOF targets and achieve target payments
  • Identify any areas of Enhanced Services which will benefit the practice and could feasibly be delivered
  • Ensure that the practice reaches its prescribing targets and achieves the target payments
B. External Communication

Liaise with a range of stakeholders including:

  • The ICB
  • Additional Roles Reimbursement Scheme funded through the PCN
  • Local Community Pharmacies (including optimising the impact of national initiatives such as the Community Pharmacy Consultation Scheme)
  • Associated services
