You will work closely with our integrated frailty team which also includes Occupational Therapists, Physios, Specialist Nurses and Geriatricians.

Our team is passionate about continuous development to ensure the best quality care and experience is provided to our most frail patients.

We don't shy away from trying new things, after all, Frailty is now nationally recognised as its own speciality due to the complexity & experience required in this field so we are all learning together too.

As you can see below, there's lots to do & get involved in and there is opportunity to work in both acute and community settings.


Our fab team, known as LIFT, although small, is strong, really supportive & we manage to retain our sense of humour despite some of the pressures we face together and this is mirrored in the wider teams in which we work.

Although a relatively new unit - the Acute Frailty Assessment Unit has received extremely positive feedback. Patients describe it as a warm, caring environment where they are made to feel welcome. This is where we find job satisfaction, knowing the difference we have made to their experience and lives. We are sure that if you value patient experience, this ethos will drive you and bring you the rewards you deserve


The patient turnover is very high so satisfaction is high & days fly by, but this is an opportunity that will see you flourish.

We also respond to the Emergency Department, providing specialist comprehensive geriatric assessment, treatment & discharge management of frail patients.

We want to do all that we can to avoid unnecessary admissions and ensure safe but rapid discharge for our patients.


There will be opportunity to contribute to our integrated Hot Clinics based both in the hospital and in the community.

We also manage a community case load which we call Post Discharge Frailty Support.


We are on a continuous cycle of improvement which includes setting up a community based Virtual Frailty Ward to allow patients to be discharged sooner and provide an alternative to hospital.

We have 9000 fantastic people working hard to deliver quality services to our patients.

  • To communicate and collaborate with all Core Therapies Service stakeholders
  • To support personal and people development
  • To help to ensure the health, safety and security of users of the Acute Frailty Team, self and colleagues
  • To support service development and planning
  • To support the Clinical Governance and Quality agenda within Core Therapies
  • To support equality of opportunity and access within the Core Therapies for both staff and patients.
  • To support financial and other resource management within the Core Therapies
  • To provide and use information to support admin processes and patient care
  • To directly support the delivery of therapy to patients
