An exciting opportunity has been created for a Mental Health Practitioner to work within the Miles Platting and Newton Health Primary Care Network across Manchester.

The post holder will work alongside the Senior Mental Health Practitioner to provide a combined consultation, advice, triage and liaison function, supported by the local community mental health provider and will work with patients to:
(1) support shared decision-making about self-management; (2) facilitate onward access to treatment services; (3) provide brief interventions, where qualified to do so and where appropriate.


The Mental Health Practitioner will work with the Senior Mental Health Practitioner to offer a clinic to assess the mental health and social care needs of individual at the request of the GP.

As part of the wider Primary Care team, you will promote and implement early intervention, avoiding unnecessary medication, referrals and hospital admissions, and support service users to self-care, whenever possible.

You will be a conduit between primary care, Living Well, secondary care, VCSE organisations and other agencies ensuring warm introductions and smooth seamless journeys for service users.

The Mental Health Practitioner will carry a caseload of service users for time-limited work.

The role will mainly work with people who need specialist mental health team care, those who are not responding to primary care mental health interventions or are not appropriate/not ready for IAPT, thus widening access to mental health support across services.

The role will be based in the Newton Heath and Miles Platting PCN.
