The Derbyshire Federation for Mental Health (DFMH) is a charity supporting individuals living with or recovering from mental ill health.


We are looking for an inspirational individual to join us as Service Coordinator, within the new Young Adults Transition Service (launching initially in the Chesterfield area). The service will be a partnership between DFMH and Action for Children; it is designed to ensure that young people leaving the care of CAMHS or other statutory service provision will be adequately supported to transition out of those services with the skills and resilience to maintain positive mental wellbeing.


This new innovative service has been designed to support the transition of young adults from CAMHS or Adult MHS into a social model of support.


Young Adults Transition Coordinator

£ 25,350 per annum pro rata to 37.5 full time hours

(plus 3% pension contribution).
**28-35 hrs per week

  • Weekly working hours negotiable.**This role is an essential part of the Young Adults Transition model. As Service Coordinator you must have substantial experience in managing teams, working with young people including those living with or recovering from mental ill health. The service will deliver evidence based, early intervention, targeted support within a designated region of Derbyshire and experience of this work is essential.


You will have experience of engaging young people and have a clear understanding of resilience strategies and the use of evidence based techniques to aid young people to strengthen their ability to maintain positive wellbeing.

Use of a car and full driving licence is essential for this position.
