Description

As a Care Navigator, you will provide support to patients in Enfield as part of a new joined-up Multi-disciplinary (MDT) service and the Single Point of Access (SPA).


The service involves providers across several sectors:
primary care, community health, mental health, hospital, social care, and the voluntary sector.

The purpose of the service is to provide holistic care for people living with multiple Long-Term Conditions (LTCs) and other additional needs.


You will work within a multi-disciplinary team and will collect information to support individual holistic assessments and direct patients to appropriate services including activities that may help to promote their health, wellbeing and independence.

The MDT service is in development, therefore the list of responsibilities below is not exhaustive, as the service develops you may be required to take on additional duties:

You will work within a core team collecting information for the holistic assessments and providing a plan to those consenting to the MDT process.

You will be required to use multiple systems such as EMIS & RIO, amongst others to carry out assessments and log information.

Your aim will be focusing on helping patients to make full use of local community and voluntary sector resources available locally.

Establish and maintain relationships with the voluntary and community, statutory and other agencies so that you can respond holistically to patients' needs.

Build relationships with the wider social prescribing community in Enfield to benefit the people you support.


Working with the MDT service members to increase their awareness and use of local services, develop new opportunities, and problem solve to improve access.

The MDT service is in development, therefore the list of responsibilities below is not exhaustive, as the service develops you may be required to take on additional duties:
To be able to work independently using own initiative, with mínimal direct supervision and escalate issues outside areas of competence
To be aware of personal accountability and responsibility for cleanliness standards and Trust infection control policies.
To ensure the smooth and efficient running of the MDT process.
