Description

This post will contribute to the health improvement of the population by supporting people to make a lifestyle behaviour change.

The post holder will contribute to the delivery of the Live Life Better Derbyshire service.

This will involve supporting clients to identify and address their holistic wellbeing needs including the direct provision of weight management, physical activity and smoking cessation support.

The service is aimed at individuals aged 16 and above (stop smoking aged 12 and above), and families with children under 16.

Key Relationships:
This role reports to the Service Manager The role provides a service or support to: Clients of the service and the target population of the service A broad range of partners including health professionals, district & borough councils, voluntary sector, businesses and local communities
