Crisis Team and Home Treatment Practitioner - Band 6 full time permanent post.


Rotherham Doncaster & South Humber NHS Foundation Trust is seeking to appoint an additional Band 6 Mental Health Practitioner (RMN/SW) to work in the Rotherham Home Treatment Team and the Crisis Team.

Your role will be in Borough Wide mental health services providing urgent and emergency response.

This includes telephone triage and support of callers to the crisis line, and assessment and short-term care planning for people experiencing a mental health crisis in the Rotherham locality.


The Home Treatment Team provides an intensive home-based alternative to hospital admission, and also facilitates an Early Discharge pathway from the Acute wards.


The post will involve flexible duty rota covering seven days a week throughout a 24-hour period, with your primary base being at Woodlands, with the possibility of some cover at Woodside.

This is where the out of hour crisis calls are taken and is close to Woodlands itself. The Home Treatment works core Hours of 9-7 across the 7-day week. The post is a combination of both teams, all the Home Treatment team work across into Crisis.


You will need to demonstrate relevant experience of working in a multi-disciplinary setting across mental health services and with other partner agencies.

Working within Urgent mental health services is desirable but not essential as we can support you to develop in this field and you will most likely have transferable skills.

The team is very supportive of new staff members.


You will possess excellent clinical triaging and assessment skills using a Biopsychosocial model and be able to work effectively with service users and their relevant others.


Knowledge and understanding of PDR and supervision processes are also essential requirements for the post together with a commitment to the appraiser and supervisor role.

We promote regular supervision and team discussion / debrief as part of our working culture.


Additionally, you will need to possess excellent communication skills, be motivated, able to work independently, and as part of a multidisciplinary team and with other partnership agencies.

You should have a positive approach to meeting the needs of our service users and be able to demonstrate continued professional development
