We currently have exciting opportunities for motivated and experienced professionals to join our team as CHC Practitioners. Our service operates Monday-Friday 9-5 and we have both Full Time and Part Time opportunities available with a range of flexible working options.

The successful applicant will manage a caseload of CHC eligible individuals in accordance with National Framework for Continuing Healthcare, Undertake eligibility reviews and assess individuals needs in order to identify most appropriate care options. You will assist NHS Dorset commissioners in searching for care and make recommendations on most appropriate placement.
**You will work within a strong and supportive team, playing a key role in making recommendations on eligibility for CHC as well as supporting the Operational Managers in the implementation of the national frameworks and other national policies as they relate to CHC/FNC.
