The search has begun for a Team leader within the Planned and Proactive Care Directorate; Community Neurology, Mid Nottinghamshire & Bassetlaw We are looking to recruit an enthusiastic, creative and driven Team Leader to support and guide community nursing/AHP teams.

You will need to possess community nursing/AHP knowledge, current and expert knowledge on the service, be aware of national drivers in community Neurology services and demonstrate compassionate leadership that meets the NHS Model of Leadership.

As the Team Leaderyou will:

Oversee care approaches and systems of delivery and have clinical oversight and day to day operational management of the clinical teams, leading on CQC compliance, governance, assurance, recruitment and retention, roster oversight, management of staff issues, leading training and development and ensuring that our care delivery is exemplary.

Lead and co-ordinate a high quality, safe, effective and efficient service to meet the needs of the defined patient group through support, kindness and respect.

Be a central figure of providing strong leadership throughout the wider multidisciplinary team ensuring that high standards are maintained and improved upon with the support of the Head of Service and General Manager.


Work closely with key stakeholders within and external to the service to drive improvement and maintain the profile of the community Health Services.

Developing and implementing new initiatives to ensure that the experience of patients and those important to them is of the highest standard.

Be an ambassador and representative for Nottshc at placed based partnerships and ICS level meetings You must be an NMC/HCPC Registered clinician with Neurology experience and evidence of continuing professional development and knowledge of working with people with complex physical health problems, to inform and support the clinical care pathways; preferably within a community setting although clinical skills and experience are transferable.

You will need to be highly motivated, have positive effective interpersonal and management skills and a commitment to promote recovery and rehabilitation orientated practice with the aim of preventing unnecessary and reduce hospital admissions in a variety of ways.


A full UK driving licence and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.
