Team Leader - Domiciliary Care
£300 Welcome Bonus
CDA Care are currently recruiting for a Team Leader to manage a team of Care Workers, Coordinators and Senior Staff.
This is an ideal role for an experienced Domiciliary Care Team Leader or a Care Coordinator who wants to progress their career to the next level.
Duties
Your duties will include completing outcome based assessment with new service users and creating a person centered support plan and risk assessments.
You will be responsible to work in partnership with Health and Social Care professionals to benefit from pooled expertise and resources to ensure our service users achieve their desired outcomes.
You will conduct frequent reviews and be point of contact for our service users, families and stakeholders. It is essential that you build a good working relationships with all stakeholders.
You will identify training / development opportunities and conduct frequent supervisions and unannounced spot checks to support staff with ongoing development.
When required you will be expected to support Senior Staff and Coordinators to ensure continuity of care and services are delivered safely.
You will ensure that the area is compliant with all regulatory requirements and be expected to work towards strict deadlines.
You will implement our policies and procedures and ensure that the Social and Wellbeing act 2014 is embedded within the service.
You will be expected to complete ongoing personal development and Register with Social Care Wales.
Your shifts will be Monday to Friday 9am to 5pm however must be available at short notice from 7am to 10pm to ensure a safe delivery of service.
Skills, Qualifications and Experiences
- You must have a full driving license and access to a vehicle.
- Experience within a senior role and domiciliary care setting
- Successful a DBS check and reference checks.
- A minimum of a NVQ or QCF level 2, or 3 in Health and Social care is required
- Experience working with other health and social care professionals to ensure best outcomes
- Be able to have a good working relationship with your team, identify training / development opportunities and experience with supervisions
- Be flexible
- Approachable and a peoples person
- Excellent I.T and administration skills
- Excellent listening, speaking, reading and writing skills.
- Preferred experience with Electronic Call Monitoring and Rostering systems
