    DESCRIPTION of the Homecare Pharmacist Role:
    You will be working alongside our Principal Pharmacist Medicines Optimisation in shaping the future of healthcare for our patients by playing a pivotal role in helping to deliver on the NHS 10-year health plan of shifting health and care from hospital to community.

    Under the leadership of the Principal Pharmacist Medicines Optimisation, you will work with a wide range of stakeholders, including pharmacy teams at The Christie, collaborative partners at The Christie Pharmacy, Baxter Healthcare, peripheral and outreach service satellites across the local cancer network, and NHS England, to optimise medicines across the patient pathway. This includes commissioning, procurement, distribution, and patient delivery, ensuring that resources are used wisely and effectively to improve health outcomes and reduce health inequalities.

    Key Responsibilities:
    • Manage, monitor, and develop Clinical Pharmacy Services for Homecare initiatives.
    • Implement strategic direction for pharmaceutical services, focusing on high-cost medicines and homecare services.
    • Develop high-cost medicines optimisation strategies, including setting standards and monitoring performance to ensure safe and efficient service delivery.
    • Support the Principal Medicines Optimisation Pharmacist in developing, managing, monitoring, reporting, and promoting the Trust's medicines optimisation programme, maximising value for money and cost-effective prescribing.
    • Develop expertise in medicines optimisation, including initiating and facilitating pharmacy practice research in homecare and medicines value.

    Job Responsibilities:
    Medicines Optimisation (Homecare Services)
    • Act as deputy to the Principal Medicines Optimisation Pharmacist.
    • Manage all pharmacy-related aspects of homecare services, supported by the Principal Medicines Optimisation Pharmacist.
    • Develop and monitor homecare performance indicators to ensure compliance and implement action plans for improvement as needed.
    • Contribute nationally to the development of homecare medicines optimisation guidance and policy, attending relevant committee meetings.
    • Ensure homecare procurement meets Trust Standing Financial Instructions and European procurement legislation.
    • Identify areas for audit to improve patient care, safety, and experience related to homecare medicines use and provision.
    • Lead the development and review of homecare scheme SLAs, protocols, treatment guidelines, service specifications, prescription forms, and patient information.
    • Liaise with clinical pharmacy team leaders, lead pharmacists, outpatient dispensary managers, and other pharmacy staff to identify new opportunities for homecare services.
    • Work with clinical service managers for SACT and Outreach, preparing business cases, KPI reports, and presentations to finance or external providers.
    • Manage and develop homecare medicines optimisation services, providing specialist pharmacist support to the Christie at Home team and Outreach Services.
    • Lead and participate in service review meetings for homecare medicines optimisation, working closely with homecare providers, pharmacists, nurses, doctors, and patients.
    • Produce quarterly reports on quality and financial indicators for all homecare medicine services.
    • Support the Principal Medicines Optimisation Pharmacist and cancer disease groups in forecasting budgets for homecare service expansion and identifying potential savings.
    • Coordinate training initiatives and competency frameworks within the homecare pharmacy service and assist in delivering training to pharmacy and homecare teams.
    • Manage outsourced homecare-related incidents with homecare providers and third-party dispensary.
    • Manage medicine-related incidents occurring within the Christie@Home service, including those involving the third-party courier.
    • Attend meetings relevant to disease groups regarding new homecare initiatives and represent the pharmacy service at clinical unit and divisional meetings.
    • Develop and implement procedures for clinical screening of homecare prescriptions, including document approval and control.
    • Provide training for specialist clinical pharmacists in clinical screening of prescriptions for medicines supplied via Homecare.

    To support the Principal Pharmacist Medicines Optimisation:
    • Develop initiatives to provide care closer to home, including Outreach and Homecare services, through collaborative working with stakeholders.
    • Develop and implement projects to redesign services where necessary.

    Medicines Optimisation (Medicines Value)
    • Support the Principal Pharmacist Medicines Optimisation in the Trust Medicines Value Improvement Programme through identification, prediction, assessment, and reporting of savings from medicines procurement and supply chain activities.
    • Support pharmacy procurement and relevant clinical/medicines optimisation colleagues in ensuring appropriate clinical and commissioning input into medicines procurement decisions.
    • Implement national and regional procurement guidance to manage drug shortages and maximise savings for the NHS.
    • Conduct risk assessments for procured medicines with clinical colleagues.
    • Provide clinical input into medicines shortage information compiled for the wider pharmacy team and clinical colleagues.
    • Support senior pharmacy teams in managed entry of new medicines into the Trust according to National and Local commissioner guidance and Drug & Therapeutics Committee implementation considerations.
    • Develop and support initiatives to maximise savings for commissioners.
    • Provide commissioning support to clinical teams to understand and implement commissioning policies and protocols.
    • Support production, coordination, and management of horizon scanning information for new medicines, liaising with senior clinical pharmacists and clinicians.
    • Use expert clinical and commissioning knowledge to identify approval routes for individual patients.
    • Support provision of reports to DTC regarding medicines value and compliance with NICE TA and Commissioning statements.
    • Develop, validate, and monitor reporting for effective medicines usage and financial budget management.
    • Support prescribing at the individual patient level, contributing to optimal drug therapy with consideration for risk/benefit issues.
    • Deliver a clinical pharmacy service to designated patient groups, promoting best practice according to local, Trust, and national guidelines.
    • Identify and resolve complex pharmaceutical issues, considering co-morbidity factors associated with patients and liaising with medical and nursing staff.
    • Establish processes for setting standards of care, identifying risks, and implementing changes to achieve these standards.
    • Undertake clinical audit and practice research as part of the multi-disciplinary audit process within clinical pharmacy service.
    • Identify and report systemic or preventable errors associated with medicines within the Trust and facilitate learning.
    • Demonstrate safe and effective screening/review of prescriptions for chemotherapy regimens, clinical trials, and treatment strategies.
    • Support development and delivery of clinical pharmacy services to advance safe, clinically effective, and cost-effective use of medicines.
    • Ensure timely supply of medicines to patients with dispensary staff and commercial providers.
    • Recruit staff within the Pharmacy Homecare team in conjunction with the Principal Pharmacist Medicines Optimisation.
    • Regularly appraise the team and ensure mandatory training is up-to-date.
    • Plan and implement strategic goals for the Pharmacy Homecare team.

    Qualifications:
    Essential:
    • Vocational Masters Degree or equivalent in Pharmacy
    • Membership of General Pharmaceutical Council
    • Post Graduate Diploma in Clinical Pharmacy or equivalent
    • Evidence of continuing professional development
