The Prince & Princess of Wales Hospice is a well-established charity located within Bellahouston Park.

The Hospice provides specialist palliative care for people aged 16 and older living with a life-limiting and progressive illnesses as well as providing support for patient families and carers.


We have an exciting new opportunity for a Care Co-Ordinator to support young adults living with a life limiting condition.

This role will be to expand the young adult activities and sessions held within the Living Well Hub as well as to scope the potential for day respite within the hospice.

This post forms part of our developing Young Adult Services which is a new, exciting, and innovative holistic model of care which is at the forefront of young adult palliative care in Scotland.

As part of the Young Adult Service Model, we currently offer a Young Adult Transition clinic, a Living Well Hub (built on the pillars of wellbeing), Art, Family Support Services.

Following a successful pilot, we will be launching our Short Break Stay Service for 2023.


This post in addition to the above activities will be responsible for carrying out introductory visits for young adults and their families including those who are transitioning from children to adult's hospice care services.

You will be required to assist in the assessment and planning of meeting the young adult's care needs, including what is important to them whilst attending the Young Adult Service


As a registered health and/or social care practitioner you will have experience of working with young adults with complex care needs and will contribute to the development of our service for young adults, including transition between children's and adult services.

You will be required to provide support and guidance for our young adult multi-disciplinary team (MDT).


You will be responsible for supporting young adults living with progressive illness and complex health care needs to lead their best active life.

You will be working closely with individuals to ensure their overall experience during their visit is positive.


Care, passion, and compassion is fundamental to hospice care and a key part of this role,

An integral part is also being able to connect with young adults, their families, and carers.


Desirable is experience of working within a multi-disciplinary team with evidence or experience of working within a learning disability environment.


Job Types:
Full-time, Temporary contract

Contract length: 12 months


Salary:
£26,889.00-£33,092.00 per year

  • Weekend availability

Ability to commute/relocate:

  • GLASGOW: reliably commute or plan to relocate before starting work (required)

Experience:

- providing care: 1 year (preferred)
