Description
To work as a fully integrated member of the multidisciplinary team providing direct care to people using our services.

Develop positive relationships with the people who use our services, and their carers, adopting a pragmatic and non-judgemental approach and encourage and support individuals to participate in ways which support their individual recovery journey.

Contribute to meaningful care plans and risk management plans and deliver low intensity therapeutic interventions.

  • Support delivery of planned care following biopsychosocial assessments, practicing in a way that actively minimises dependency and promotes recovery
  • Deliver services using a recovery and strengths approach where the needs of the individual, and their carers, are held as central and their involvement is proactively encouraged
  • Supporting arrangements for carer assessments and referral to local authorities as required
  • Responsible for managing their own workload to ensure the needs to the individuals who use our services are met taking the wider needs of the team into consideration.
  • Plan own workload to ensure that care is provided in the most high quality and cost efficient manner, in line with appropriate policy and be able to prioritise and make adjustments as appropriate
  • Ensure that interventions are planned in an integrated and coordinated manner particularly where dependent on other external agencies
  • Support access of appropriate information and guidance around medication via internal and external sources, referring on to most appropriate agencies where necessarY
  • Maintain up to date clinical records in a timely and accurate manner
  • Ensure information is shared with key stakeholders and other agencies in a secure manner
  • Work without direct supervision and take accountability for your own actions, working within the operational policies and service specifications of the team

Working from home contracts do not attract high cost area payments.

  • To contribute to the team's engagement with a wide range of stakeholders including staff, people using the service, their carers, GP's and other statutory and voluntary sector agencies.
  • Liaise with a wide range of other professionals and agencies to ensure the provision of consistent wellcoordinated care; secondary mental health multidisciplinary teams, Primary care teams as well as external agencies also involved in the provision of care e.g. voluntary and private sector, police, social services, housing, employment etc.
  • Provide supervision and development for Healthcare assistants on the unit (Band 2)
  • Undertake physical health checks as required i.e. blood pressure, temperature, respirations, ECG and bloods
  • Able and suitably trained to provide CPR and use of Defibrillator
  • Undertake calculations of patient monies and record appropriately
  • Undertake escorts both internal and external to the unit
  • Support assessment, planning, intervention and evaluation of care packages and Direct Payments so the person is offered as much choice and control of resources as appropriate whilst focusing on integration within existing community resources where possible
  • Ensure that appropriate action is taken to safeguarding adults and children. This will include providing and receiving highly complex and sensitive information in relation to safeguarding adult and children work, coordinating and chairing safeguarding meetings and implementing relevant care plans
  • Ensuring timely and accurate documentation of all clinical activity in line with professional guidance and best practice
  • Contribute to the achievement of the key performance indicators for the Trust being met whilst providing high quality carE
