The shift pattern for this post is Monday to Friday 9-5

An exciting opportunity has arisen within Glasgow City HSCP for a Band 4 HCSW to join the Hospital at Home team.

Hospital at Home will enhance the capacity within existing frailty and existing community based services.

The Service aims to significantly reduce unscheduled care activity across the older peoples care pathway and reduce attendance at the Emergency Department, Integrated Assessment Unit, reduce bed days and reductions in delayed discharges.

The post will provide clinical, operational and professional leadership.

You will have a formal qualification at HNC level or equivalent experience in health care environment. Knowledge of the clinical speciality/area process, techniques and/or activities relevant to client group. Ability to travel independently locally, work under pressure and on own initiative. Ability to work individually or as part of a team.

This post will be integral to the development of the Hospital at Home Service
