Paying £32.45 an hour

We are looking for a high calibre qualified social worker with a minimum of two years post-qualifying experience in the field of MDT working with health partners.

The successful applicant will be carrying out Continuing Healthcare assessments with nurse specialists from the local CHC clinical assessment team - so this is an excellent opportunity to work in an integrated way with NHS colleagues.

The post-holder will also carry out assessments with residents in the community who need support to carry on living as independently as possible in their own homes, so experience of using equipment and Reablement services to promote independence is essential.

The post allows for a varied caseload, and therefore provides scope to deploy a broad range of social work knowledge and skills.
