Accountable to:
Hospital Liaison & Support Group Lead


Based at:
Home/Newick with mobile/remote and home-working and regular travel to East and West Sussex Hospitals / service venues.


Job Summary
There are two separate posts. One based in East Sussex and the other in West Sussex.

Develop the Hospital Liaison service in either East Sussex or West Sussex.


Hospital Liaison

  • Coordinate and deliver a proactive, targeted Hospital Liaison service to patients 18+ with either diagnosed or a suspected brain injury, their families and carers as directed including those with nontraumatic brain injury as a known area of unmet need.
  • Integrate the service to work with, induct, supervise, and support volunteers and students within identified hospitals to provide advice and support to people admitted with brain injury and to their families and carers to include:
  • information on Headway services with support offered to access those services e.g., Wellbeing Support Groups, specialist counselling, Carer workshops and courses.
- information leaflets, guidance and advice on brain injury and the management of symptoms
- follow-up telephone and online calls where required to provide support after discharge.
- referral to other agencies/services as required including introduction to other Headway groups and branches for out of area referrals and signposting to benefits advice and provider/partner organisations.

  • Support individuals and families to help manage feelings of traumatisation, bewilderment, and isolation particularly after discharge when coming to terms with brain injury and adjusting to significant life changes. Target followup support for patients affected by postconcussive syndrome.
  • Strengthen existing partnerships with hospitals and disability and health organisations and help build new ones. Develop effective working relationships with hospital clinical teams including specialist nurses and A&E staff. Explain and promote the service within hospitals including via team meetings.
  • Coordinate targeted provision of Headway service information within hospitals liaising with communication and marketing teams as appropriate. Manage supply of hard copy, social media information and direct communications to help raise awareness of the Headway service, brain injury and its impact.
  • Work as directed by Headway Managers to lead a rolling programme of awarenessraising across Sussex Health and Care Partnership and VCSE organisations. Source and produce relevant information to address agreed priorities;
- for example information on what patients can expect when returning home, including cognitive and physical changes.


General Service Delivery

  • Work with the Headway management, staff and volunteers to feedback client/patient requests and help to develop an engaging programme of group opportunities to improve client personal wellbeing, independence, and quality of life.
  • Liaise with volunteers, VCSE, NHS, Social Care colleagues, preferred provider partners and other activity leaders to reduce duplication of effort and prevent confusion for patients and carers.
  • Maintain a safe, friendly, supportive, and high standard service environment. Undertake risk assessments and follow agreed actions. Ensure service delivery meets Headway agreed quality standards.
  • Work with the wider Headway team to publicise and promote the Hospital Liaison programme integrating messaging within the wider Living Well programme media framework. Contribute regular content for social media platforms and other materials and communications.
  • Work closely with Headway Living Well Programme Leads and managers to achieve continuous improvement. Help Headway develop new wellbeing group support influenced by the needs and views of clients and carers.
  • Share best practice across the wider Headway team. Work with the Programme Leads and Community Service Manager to set up an effective monitoring and evaluation process to include routine client feedback.
  • Contribute to preparation of policies, guidance, and procedures for safe, effective operations.
  • Consistently promote a partnership ethos.
  • Focus on meeting client needs at point of contact with Headway, promoting choice and control, personal wellbeing, rehabilitation and reablement, independence, improved quality of life, social interaction, and inclusion.
  • Liaise with Headway Response Line and Call Back Service to ensure any new enquiries from clients or carers are coordinated through the system and not overlooked.
  • Promote and implement Headway procedures to safeguard the wellbeing of vulnerable adults and protect them from abuse.
