North West Anglia Foundation Trust (NWangliaFT) recognizes the contribution and support unpaid carers provide for patients with heath and social care needs whilst in their homes and as patients.
Supporting those who provide unpaid care is therefore hugely important for not only the carer but also our patients.
In 2014 NHS England published their commitment to carers which detailed a framework of best practices to identify and support unpaid carers.
We are looking for a new Carers Advsior to Join the Trust based in Hinchingbrooke Hospital who as part of the wider patient experience team will identify carers in the Trust, discuss their individual needs and signpost them to additional support/provider (including carers assessments), raise awareness of the valuable role unpaid carers through education and training whilst supporting the discharge process through a person centered care approach.
We will support you through induction into the new role & offer ongoing dedicated ongoing supervision & support. The option to explore training and development you may need to begin a fulfilling career will also be available
