Description
This is a new and exciting role to provide a service for young, fathers who experience health and social inequalities and whose partners are eligible for the Family nurse partnership (FNP) service in Bristol.
Overall, the aim of your role will be to improve health and wellbeing outcomes for the young men themselves, their babies, and their families.
This will be achieved by offering one-to-one visits to young men, from before the birth of their child up to when the child is one-two years old.
You will compliment and work very closely with the Family nurse partnership team to achieve the outcomes.FNP is a national evidence-based, intensive, home visiting programme for young, vulnerable women and their families, from pregnancy and up to two years.
The aim of the programme is to improve the mother's antenatal health, child health and development and parental economic self-sufficiency in disadvantaged young families.
You will work alongside the team to offer targeted support to the partners of clients enrolled onto the programme.To raise the profile of young Fathers and to enhance the service offer for Fathers across Bristol.
Establish and maintain effective networks with statutory and voluntary services that support young Fathers.
Support/signpost Fathers to access healthcare/financial support/housing/training/employment, to reduce inequality in access to services.
Establish a therapeutic relationship with young Fathers, using strength-based solution focused strategies and motivational interviewing to enable behaviour change.
Support the development and delivery of specified 'Support Packages' to young Fathers, which are short term, time limited interventions.
