Description

1 Team leadership 1.1 To provide clinical and professional leadership for the PCN Pharmacy Team 1.2 To ensure that the our Pharmacy services are patient centred.

To ensure that patients are actively engaged in the planning and delivery of their care.

To promote the voice of patients and carers and to ensure that feedback, complaints and other representations from patients are thoroughly considered in the planning and delivery of services.

1.3 To work with our Pharmacy Team and Management Team to develop and implement strategies which promote excellent patient care and evidence based practice.

1.4 To provide effective clinical leadership for our Pharmacy Team.

To promote confident and autonomous practice and to enable members of the Team to progress in their own personal development.

1.5 To facilitate engagement across our Pharmacy Team to promote a collegiate and high-functioning team. To foster strong levels of autonomy within the team and its members.


1.6 To promote an environment where issues and challenges are addressed collectively and team members share responsibilities to lead on agreed areas of work.

1.7 To provide a strong clinical presence and to be highly visible within our clinical operations across member practices sites.

To provide a key point of contact on a day-to-day basis, offering expert pharmacy advice to other members of the Pharmacy and wider clinical teams 1.8 Demonstrate and model positive behaviours that are aligned with our values.

To support and encourage these behaviours across the Pharmacy and wider clinical team 1.9 To provide effective line management for the Pharmacy team members 1.10 To promote job-satisfaction and well-being within the Pharmacy Team through providing compassionate pastoral support.


1.11 To ensure and provide suitable regular scheduled and ad-hoc clinical supervision for the Pharmacy team members 1.12 To work with the Pharmacy team both as a team and individually to develop confidence and competencies 1.13 The ensure that all members of the Pharmacy team: Are supported with the right training, equipment, space and support to deliver their roles effectively Deliver high quality personalised care Adhere to all our relevant operational policies and procedures Operate within their competencies and limits of their registration Operate within the parameters of their agreed job description and employment contract Operate in accordance with our regulatory and contractual requirements Operate in an efficient way that promotes the responsiveness and patient access Undertake appropriate mandatory training as prescribed Maintain appropriate care and other records according to relevant policies 1.14 To undertake (or oversee) individual appraisals of all members of the Pharmacy Team including agreeing and supporting development plans.

2 Clinical Practice 2.1 To work clinically as Pharmacist with some degree of advanced practice and/or with specialism,.

The balance between Pharmacy Team leadership and the postholders clinical practice will be established via an agreed job plan to be determined and documented at the time of appointment.

This job plan will be kept under review and may be adjusted by mutual agreement between the postholder and their manager.


2.2 Functioning with a high degree of autonomy in their clinical practice, receiving clinical supervision from the Pharmacy lead GP on a regularised and ad-hoc basis 2.3 To remain clinically competent in relation to their job-plan, registered with the GPhC.

2.4 The postholder will ensure participation in their own agreed Personal Development Plan and will progress the development of their own skills and competencies according to this.

3 Operational management 3.1 To oversee the day-to-day operations of the Pharmacy Team 3.2 To oversee the allocation of resources within the Pharmacy services, including overseeing the rota, and ensuring the effective delivery of Pharmacy functions within the member practices.

Develop a level of resilience within the Pharmacy team and processes to minimize the impact of any absences, both planned (e.g.

Annual Leave) and unplanned (e.g. sickness). 3.3 To work with our Management Team to contribute to our workforce plans. To lead for the recruitment of Pharmacy staff as required.


3.4 To support the delivery of good prescribing practice within our clinical operations and alignment with CCG Formulary and relevant prescribing incentive schemes.

To have a role in overseeing our performance against commissioner prescribing incentive schemes.

This will include supporting and supervising medicines management functions within the member practices and supporting our response to medicines alerts and other relevant notices.
