Hospital Discharge Social Worker

Tameside
Hybrid Working £35 Per hour

Tameside are seeking experienced Qualified Social workers to join an existing team of friendly, skilled, and experienced staff. The role is paying up to £35 per hour with the flexibility to work where you can be most productive 3 days a week with 2 days inthe office

Contract Length:
Ongoing

Job Duties:

What will you be doing? You will be responsible for managing your own workload and working hours to ensure all your work is completed on time.
Maximum caseload of 20

*Ability to deal with complex discharge planning including completion of mental capacity assessments and making best interest decisions

Providing and contributing to meetings and reports.
Dealing with Safeguarding concerns
To support and vulnerable persons to be discharged safely from hospital back to the home environment with appropriate care and support.
