Description

We are looking to recruit a Health Care Coordinator alternatively it would suit a Health Care Assistant (Band 3-4 dependant on skill set) to become part of The City of York Primary Care Network (PCN) Care Coordinator Team.

This exciting new role is an evolving area of care mapped out by the NHS Long Term Plan.


This will be achieved through areas such as supporting the patient to, access available services; running Multi-Disciplinary Team (MDT) meetings to link up available aspects of their care; performing reviews within the patients home and ensuring fluidity between Secondary and Primary Care services.

Work as part of the Multi-Disciplinary Team and ensure fluidity between care provided by services to your patient.

Support, implement and review individual care plans, self-management plans and to agree trigger thresholds for escalation.


Provide coordination and support for people and their carers across health and care services, closely working alongside social prescribing link workers and other primary care roles.

Liaise with services to ensure timely and appropriate engagement as required.


Identify patients that are of higher risk of admissions to hospital and support them in areas that can help to reduce this outcome.

Organise, attend, and document MDT meetings as appropriate and act as deputy chair as needed.


Maintain accurate and up to date records of people's contacts using GP record systems and other IT systems relevant to the role.


Undertake visits or arrange appointments to perform health reviews as required which may include tasks such as blood taking, and taking observations.


Follow up on communications from out of hospital and in-patient services regarding changes in condition of patients to support the practice to respond proactively to potentially unmet needs.

Monitor patients to ensure tasks are completed and care delivered.

Deliver high-quality primary health care services

Build a network and knowledge of referral routes to and from service providers.

Refer people and/or introduce them to appropriate organisations in York and nationally (where appropriate) e.g. voluntary, statutory (local authority) and local NHS organisations.

Be responsible for the organisation, planning and of own workload to meet set deadlines.

Following PCN and practice policies and procedures as appropriate.

Duties may vary from time to time under the direction of the PCN workload and staffing levels.

Full training given. Driving licence essential.
