Purpose of the Job


To ensure that the company philosophy of excellent care and continuous improvement is embedded in the culture out on the patch.

To ensure that care is completely person centred by the provision of accurate and up to date support plans involving the client in the development of this plan every step of the way

To represent the company's best interest and reflect the values of the organisation in all dealings and communications with both staff and other professionals


To ensure robust health and safety policies on the patch are adhered to and risks are identified and managed appropriately.

To conduct good communications with the office and in particular care - coordinators / senior supervisor and to ensure customer satisfaction and that client's needs are reflected in their scheduled calls


To contribute as a member of the senior team to the meeting of Lakeland Care Business Plan targets which are based upon our 4 main objectives.

Key Tasks (Not a definite list)


To work across the WHOLE of the patch as required on care as a key part of the job to ensure that an in depth knowledge of clients and staff is maintained and to improve continuity when there are times of sickness.


To conduct support shadowing to new members of staff to ensure they are comfortable and confident in their role and delivering a person -centred service.


To support new staff intensively during the first three months to ensure they develop required standards of competency over their induction period.

To assist and support staff with their vocational qualifications such as Care Certificate, NCFE and Diplomas

To conduct/ attend team meetings as required.

To coordinate the activities of and support to the senior carers / supervisors.

To identify staff training needs via shadowing and competency observations

To conduct spot checks and competency assessments at the required frequency

To be back up for the client rostering system, working towards (after training) being competent to do the support workers rosters and client schedules

To ensure effective communications are maintained with other colleagues in both office and field settings

To undertake quality auditing as required

To be flexible in working hours this may change at short notice in accordance to the business needs

To work alternate weekends, including care, on call and standby duties

To carry out Client reviews and initial assessments at the required frequency, as directed by Care Manager

To contribute to the accuracy and development of client support plans


To ensure quality services are delivered out on the patch by robust quality assurance of working practice and addressing of any issues observed by one to one support and / or additional training where appropriate.


To ensure that client concerns are attended to as a matter of priority and that they are dealt with in a spirit of openness, honesty, welcoming feedback and being willing to make improvements when required.


OUT OF HOURS SUPPORT


Provide information advice and support to Care & Support workers outside of normal office hours as agreed on a rotational basis, this is likely to involve liaison with families and other professionals such as emergency out of hours team, district nurses, etc.

Provide care and support to clients if no other members of staff are available

Provide handover information to office staff and the end of each shift

Vocational Qualification level 2 or in progress.
