Description

The majority of patients identified for this service are likely to be housebound and/or frail who have unmet health and social needs.

The CWN housebound visiting service will visit these patients in the community and support to enhance quality of life.

The main aim of the service is to work with housebound and frail elderly patients to proactively identify care needs before the patient is in crisis, helping to prevent admissions, improve independence and ensure patients wishes are taken into consideration at all times.

Job Purpose The Lead Clinician will focus on assessment and proactive care management, ensuring that patients are supported to:

maximise their independence and participation in daily life; minimise the risk of crisis situations, such as unplanned hospital admissions; and overcome the barriers to engaging with services such as social prescribing and health and wellbeing coaching Main requirements of the job You will be a highly motivated qualified clinician, ideally working in any of the following roles;Advanced Nurse Practitioner (ANP), Paramedic or Physician Associate, with a passion for providing excellent patient care.


This is a fantastic opportunity to be part of a new service, alongside a team of colleagues providing proactive care to support patients to have quality of life and appropriate care at the point of need.

You will have experience of working in a healthcare environment, including physical and/or mental health settings, be able to communicate with patients, carers and families and able to function effectively as part of a multi-disciplinary team.

You will be familiar with IT tools and healthcare systems, e.g. EMIS, and well versed in using templates and reporting tools.


The network adopts new methods of working via an extended clinical workforce which includes Salaried GPs, Advanced Clinical Practitioners, Clinical Pharmacists, Practice Nurses, Social Prescribers, First Contact Physios and Physician Associates.

This is an exciting developing service with all seven of the practices within CWN Primary Care Network working together to achieve the aims of the project.

The role will be mobile, with a main practice base, and will cover the whole PCN area, drawing patients from our practices who live in an area ranging from Greenbank Road to Hartsbourne Avenue in South Liverpool.

As practices we are jointly committed to creating and maintaining a fair and supportive working environment and culture, where contributions are fully recognised and valued by all and staff feel empowered to carry out their duties to the best of their abilities with excellent patient care being at the heart.


Clinical supervision will be provided to the job holder in their employing practice and the job holder will benefit from regular peer support and team meetings.
