Do you have a particular interest in Urgent Community Response? We are looking to enhance our ability to deliver Urgent Community Response across our localities in Bristol and North Somerset.

This is an exciting opportunity for Therapists to work as part of our multi-disciplinary Integrated Network Teams with a focus on delivering urgent care.

You will be required to respond to urgent referrals in need of complex management in order to prevent hospital admissions and facilitate early discharges.

You will have continuing responsibility for proactively monitoring and managing patients on the caseload to ensure the best outcomes.

There will be an opportunity to develop your skills in physical assessment and clinical reasoning with the support of Advanced Clinical Practitioners and wider members of the team.

We deliver an 8am-8pm, 7 days a week service and working patterns are rostered to be able to provide this.
