Description
Working within the CAPE team, you will provide holistic, strengths-based 1:1 support to a caseload of clients. This support will primarily be face-to-face at CAPE but also include phone and video work where appropriate. When working in an outreach capacity, you will meet clients in the community for example at their GP surgery.

The role will involve you delivering both psychoeducational, well-being and therapeutic groups, including Interpersonal Effectiveness Therapy, Behavioural Activation Therapy and Compassion-Focussed Therapy for Psychosis.

You will also lead on an outreach project, working with and from GP surgeries to support patients with serious mental illness to access physical health checks.

The role of Psychological Support Practitioner is a fundamental role within our mental health team. We work with individuals with complex needs, and our clients have diverse life experiences and backgrounds.

Main Duties and Responsibilities

  • Provide support to CAPE clients
  • Manage a caseload of clients, working with the Clinical Lead to consider risk alongside support.
  • Support delivery of a range of therapy and wellbeing groups.
  • Monitor and implement strength-based plans with specific targets and timeframes for action alongside individual clients using the Mental Health Recovery Star tool.
  • Help create and deliver a variety of well-being and holistic support groups to engage clients and encourage further awareness of support
  • Build and maintain relationships with clients with complex mental health needs such that they can build on their strengths and lead more rewarding lives.
  • Build relationships with GP surgeries and support them to engage clients with serious mental illness to access physical health checks.
  • Build relationships with other organisations who share our vision
  • Set priorities and monitor allocation of time; organising and planning own workload.
  • Provide cover when required for other members of staff.
  • Support the fundraising arm of the charity.

Pay:
£29,584.00 per year
