Wellbeing Practitioner - Various Services


Purpose of the post
We are looking for passionate and committed people for this role to work across our multiple services. We have full and part-time hours available, and you will take on the role of supporting people across the Bradford District

& Craven to improve their mental health and stay well.


Key duties

  • Provide onetoone telephone and facetoface support to clients from a variety of backgrounds with varying degrees of mental health difficulties.
  • Have a good understanding of support services across the district and signpost/complete onward referrals for clients to relevant services for additional support.
  • Complete individual assessments and outcome measures with clients.
  • Develop clientcentred support plans to help individuals stay well.
  • Record information electronically and maintain up to date client records.
  • Work effectively with coworkers
  • Motivate and encourage clients to take steps to achieve positive change in their lives
  • Accept and work within the organisations policies and procedures and assist in promoting the organisations aims and objectives.
  • Have an understanding of and commitment to equal opportunities and diversity.
  • Commit to training and selfdevelopment and to undertake any training required to carry out the duties of the post.
  • Undertake any other reasonable duties as required to ensure the smooth running of Mind in Bradford.

Person Specification

Knowledge

  • A minimum of 5 GCSE's (or equivalent) grades A-C
  • Understanding of the key issues within mental health, and the potential challenges that people may face
  • Knowledge of other relevant health & social care organisations across the Bradford District & Craven
  • Knowledge of risk management and health and safety issues
  • Understanding of safeguarding and good practice
  • Understanding of confidentiality and data protection

Experience

  • Experience of working with children, young people or adults in a health, social care or any other relevant setting (this can include voluntary experience)
  • Experience of supporting people in a clear, meaningful and nonjudgemental way either via telephone or in person (this can include voluntary experience)
  • Experienced and confident in managing challenging situations, and recognising and responding to safeguarding concerns (this can include voluntary experience)

Skills

  • Excellent proven communication and organisational skills, including an ability to encourage and engage people in ways to make positive changes in their lives.
  • Excellent IT skills, including use of Microsoft Packages
  • Ability to work as part of a team and work alone and use own initiative
  • Ability to gather relevant information to support and assess risks
