Introduction


As a Care coordinator for Our Family Home Care, you will be responsible for the care services, in your area, provided by our carers and senior carers, to our clients, in line with compliance with relevant legislation and policies.

Working in partnership with colleagues in the senior management team you will ensure the provision of a high-quality service enabling individual needs to be met.


Our Family Home Care is a caring, forward-thinking company and you must be committed to developing and enhancing the services we provide.

This role is also in combination with the Carer and Senior Carer role.


The Care Coordinator must have a minimum of a level 4 in health and social care or be prepared to work towards it, leadership and or management qualification, have good communication skills, and have at least a minimum of 2 years previous experience in a supervisory role.


Job Purpose

  • To create and maintain good professional relationships with carers and all staff on induction to Our Family Home Care
  • To actively support other carers and staff throughout their employment.
  • To adhere to all regulatory and statutory obligations and Our Family Home Care policies, procedures, and guidelines
  • To actively market Our Family Home Care and promote a positive, personal, and professional profile, ensuring the good reputation always.
  • To complete staff supervision for all carers and senior carers on a regular basis, ensuring verbal contracts are completed, as per supervision policy.
  • To be on call for out of hour services, for when staff need to report any concerns.
  • To fill in for the Registered Manager when required
  • To ensure the smooth running of the team with support from the senior management team.

Responsibilities:

  • Recording and reporting
  • As per carer and senior carer when required.
  • To maintain accurate records of staff supervision, noting when they have taken place and raising any issues to the Registered Manager.
  • To complete the rota for staff within the area covered and fill in or find cover if sickness or absence occurs.
  • To overview assessments, risk assessments and all records involved when taking on a new client.
  • To overview and review clients, care plans, highlight when reassessments are required and audit when required.
  • General
  • To dress appropriately, wearing uniform and using personal protective equipment provided by Our Family Home Care
  • To seek out best practice and look at innovative ways to improve the quality and efficiency of service delivery
  • To attend and participate in regular team meetings and any other relevant meetings
  • To attend in house and external training pertinent to the role of Care Coordinator
  • To observe all health & safety rules and take reasonable care to promote health and safety of self and others and raise any concerns
  • To aim to ensure all care staff have equal treatment and equal access to services and employment rights
  • To supervise and support all new care staff through the induction period and programme, and ensure all new care staff complete the induction programme and are signed off
  • Any other duties requested by Senior Management, which are within the scope of the post
  • Adhere to all policies and procedures by Our Family Home Care
  • To cover any sickness or absence at short notice, as and when required.
  • To be responsible for the oncall phone for both clients, client's family's, carers, and other healthcare professionals, to contact on a rotation system
  • Special conditions attached to post
  • Flexible working, as evening and weekend work is required
  • Have access to own transport

Salary:
From £26,000.00 per year
