This role is based within our Care Leaver Homeless Prevention team in Lincolnshire.
You will work with Care Leavers aged who are homeless or at risk of homelessness and/or need additional support to sustain their accommodation.
Through a creative and caring approach, you will build good, trusting relationships, supporting them to find and sustain accommodation.Key Responsibilities
- To work with Care Leavers, who maybe in temporary accommodation, on NESTs waiting list, awaiting a permanent local authority property, are living within Intensive Housing Management services or are unsuitably housed for another reason
- Undertake initial needs and risk assessments of Care Leaver presenting as needing a service and establish a relationship with their Leaving Care worker.
- Deliver support to Care Leavers as dictated by their needs and risk assessment either face to face, by phone, by a virtual platform or by sharing written resources developing a bespoke tenancy sustainment plan or move on action plan
- Complete necessary budgeting work, depending on the needs of the Care Leaver, based on realistic expenses and costs involved with setting up and running a home independently.
- Review and update plans and complete regular joint meetings with the Leaving Care worker and any other external agencies involved.
- Support the Care Leaver until such time they are confident they no longer need our support and their accommodation is secure
What we need from you: - To be approachable and have the ability to build rapport and trusted relationships with Care Leavers, colleagues and outside agencies.
- An understanding of the support needs of Young People who typically use our services.
- Knowledge and understanding of housing provision and the welfare benefits system and housing legislation.
- Previous experience of working with Young People or extensive experience of providing care and/or support services within another social care setting.

A Full Driving Licence and access to vehicle are essential for this role. Travel is required throughout Lincolnshire for which a mileage allowance is payable_
An Enhanced and Barred DBS check is required for this role.
