Internal MPFT applicants only*We have an exciting and rare opportunity to join the East Staffs Community Therapies and Falls Team in Burton on Trent.

The team is based at Anglesey House but work across the whole of East Staffordshire providing rehabilitation to people in their own homes.


The successful applicant would be working alongside Physiotherapists, Occupational Therapists and Specialist Nurses to provide patients with rehabilitation goals and treatment plans and contribute to the ongoing development of the service.


We are uniquely co-located at Anglesey House with all our Community teams and work collaboratively with the patient at the centre of their care.


The successful applicant will be required to:

  • Assess, plan, deliver and evaluate delegated rehabilitation within clear protocols
  • Support the implementation of intervention/therapy programmes developed and agreed by qualified staff
  • Educate patients and their carers in the safe use of assistive aids and equipment
  • Liaise with community and voluntary services, signposting where appropriate
  • Deliver health promotion and selfmanagement advice
  • Support in the completion of patient records, ordering equipment and data cleansing
  • Maintain clear and accurate patient records
  • Demonstrate the vision and values of the organisation
A car driver is essential for this post due to the nature of our work.


You will be part of the Community Therapies and Falls Team providing high quality rehabilitation services to patients with a range of physical health conditions, following discharge from hospital, promoting self-management and preventing further admission.


You will require excellent communication skills, to be able to communicate with patients with complex communication needs, to be able to communicate with the Multidisciplinary Team, and wider NHS teams and external agencies such as care agencies.


The role involves daily documentation in electronic patient notes and the regular recording and analysing of outcome measure data and the completion of discharge summaries.


The Assistant practitioner role supports the work of a range of different professions ensuring that the individuals' physical, psychological and emotional needs and preferences are met.


This is a dynamic post that will be evaluated and adjusted to reflect the needs of the service, local population and feedback from users and staff, including post holders themselves.


You will be expected to actively participate in the development of the post to ensure it meets the needs of individuals receiving the service and their carers.

Clinical Responsibilities

  • To carry out and progress rehabilitation programmes set by qualified therapists.
  • To set and review SMART goals for individual patients
  • To discharge appropriate patients from the service, following appropriate pathways, completing appropriate onward referrals, completing timely discharge summaries and to signpost to community and voluntary groups.
  • To educate and empower patients to adopt self management strategies
  • To lead and manage neurological exercise groups, implementing changes to meet service demands.
  • To be able to complete basic observations for a patient (eg. NEWS 2, blood pressure, SaO2), to be able to recognise an acutely unwell patient, and to be able to escalate concerns appropriately.
  • To assess equipment needs, order and review equipment, and complete appropriate risk assessments.
  • To complete clinical outcome measures during 1:1 sessions, and MDT review sessions.
  • To undertake planned lone visits, routinely monitoring risk factors which may include:

  • Provision of follow up practical support or educational informat
