Community Nursing Team Leader

  • Band 7 for The Warrens community Nursing Team


An exciting opportunity has arisen for a motivated individual to work within the Community & Specialist Nursing Division in the West Wirral Locality as a Community Nursing Team Leader, within our team at The Warrens Community Nursing Team.

This is a permanent post, 37.5 hours per week.

Working over 7 days, must be flexible.

Hours to be discussed at interview in more detail.


A main part of the role is to assess, plan, implement and evaluate nursing care to patients across the community/primary care interface in the most efficient and clinically effective way, in co-operation with other members of the health and social care teams, and voluntary and statutory agencies.

This post will be responsible for managing planned programmes of care for patients in domiciliary and clinic settings.


You will need to be able to work autonomously and as part of a team and possess excellent time management, organisational and communication skills.

You will be innovative, flexible and prepared to adapt to the changing needs of the Trust as they arise.
