This post is for a qualified ACP for our Sandwell & West Birmingham Virtual Wards.

We have been commissioned to provide the clinical wrap around care in the community for the Respiratory, Frailty and Cardiology Virtual Wards across two locations.

The intention is to deliver the wrap around support as an integrated team rather than split into distinct specialties. The consultant oversight of the wards will continue along their specialist remits.


The post holder will bring their clinical expertise to a wider team and support the system around the Virtual Wards, UCR and wider community services to facilitate improvements in care.


We also host the Hospital at Home service and have a medic co-located with the UCR team 7 days a week with access to point of care bloods, ultrasound and interventions.


You will be supported by a team of community staff who will deliver delegated interventions and carry out the intensive monitoring required for people on the virtual ward.

To be a clinical expert and senior clinician in the field of Community Healthcare.

To contribute significantly to all developments and pathways associated with the Virtual Wards.


To work with the Service Lead and wider team to continually improve the quality of care provided including the continuation of service improvement to demonstrate better outcomes for the people of Sandwell.

To demonstrate research and critical appraisal skills.

Critically analyse evaluate and synthesise research, publications, recommendations of professional bodies, specialist societies and developments within the field of community and ensure that this is reflected in clinical care and where appropriate recommendations are implemented.

Within iCares we operate a 7 day service 8am-8pm, we take it in turns to contribute to weekend working and 8pm finishes.
