    Care Co-Ordinator to join our amazing team at
    Delta Care
    Ltd in Blackpool.


We are looking for a Care Co-ordinator, who will be accountable by example, within a team setting, to promote the good name of Delta Care Ltd, providing and achieving the highest standard of quality care.

To share responsibility for monitoring and supervising staff and service users, preparing rota, and providing a smooth and efficient service.

Promote Dignity in Care, Safeguard Adults and Children, and promote good infection control standards.

Delta Care Ltd Blackpool - 24 Fleetwood Road North, Thornton-Cleveleys, FY5 4AD

Hours of Duty:


Monday - Friday: 9 AM - 4 PM, office based.

You will also be responsible for on-call duties.

Objectives:


  • To undertake duties, which are considered, appropriate to the post. Exercise leadership and promote a good relationship between staff, service users, and outside agencies.
  • To be flexible.
  • Assess the needs of service users and identify problems, plan solutions, and practical individual care plans.

Duties:


  • Be aware of Inspection procedures undertaken by the Care Quality Commission and County Council Contracts department.
  • Contribute to inspections as and when required.
  • Monitor all Care staff.
  • Oncall duties as required.
  • Work with Management to promote the company and increase business.
  • Act within Delta Care Code of Practice and in a professional manner always.
  • Coordinate care staff rotas.
  • Listen to and Support Staff.
  • Provide Care to Services Users when and if required.
  • Liaise with service users.
  • Undertake weekly telephone reviews for new and existing service users.
  • Staff supervisions and spot checks and reviews as required.
  • Monitor standards of care.
  • Maintain accurate records.
  • Write reports and Letters when needed.
  • Attend and contribute to staff meetings.
  • Keep accurate records and log information on the care package system.

About you:


  • You will have knowledge and experience of working with vulnerable adults in similar settings.
  • Excellent communication and interpersonal skills.
  • Good time management and prioritizing skills; able to assess competing demands and prioritize effectively.
  • Motivated to improve services for vulnerable people.
  • Flexible and adaptable to changing environments, processes, and procedures.
  • Literate and numerate. Able to maintain accurate records in the required format.
  • Health and social care qualification relevant to the role (i.e. NVQ L3 or CQF L3 certificate or equivalent) or equivalent relevant experience.
  • Previous experience in Care Coordinating in a fastpaced environment. (Desirable)
