Care Coordinator

Near Blackburn

I'm working with a lovely PCN who is looking for an experienced Care Coordinator or an experienced Assistant Practice Manager looking for a new dynamic role.

You'll be required to work across the other sites in their practices so this is a dynamic role,and be able to provide a high-quality service to both patients and staff.


They are offering an attractive rate of pay
rate of Band 4 salary, annual leave + bank holidays, and NHS Pension.

Ideally, you'll be Emis Web trained and have experience managing your own workload. You'll be working closely with the Social Prescribing team to ensure patients have the best service.

You'll also be a strong communicator as you'll be liaising with GPs leads and other Care Coordinators.
