Do you want to be part of a new, forward-thinking Unit supporting patients and families in the Bracknell/BHFT area to rehabilitate and return to their preferred place of care?

If so, we have an exciting new opportunity for you to join us as a discharge coordinator in our friendly, supportive and award-winning team at Heathlands


All relevant training will be provided, and there are ongoing opportunities for professional development which you will be supported to access.


  • Data input and data entry including tracking referrals and data submission to both local and national reporting streams
  • Data entry
  • Gathering of clinical information via EPIC
  • Providing updates to key partners
  • Contacting health and social care colleagues for updates about patients and their discharge planning
  • To communicate effectively with both internal and external staff to ensure a multidisciplinary approach to patient care
  • Facilitate communication between inpatient and community settings
  • Build and maintain positive working relationships with colleagues in health and social care as well as the voluntary sector across the ICS footprint
  • Within specific guidelines and following training, to be responsible and accountable for your caseload, including accurate and timely documentation
  • To seek advice from senior staff within the team when necessary
  • To escalate concerns where required
  • To prioritise workload and be able to be flexible in the role as the need arises
  • To work as part of a wider team, supporting other services as appropriate where needed
  • Develop skills and knowledge to share information with members of the MDT to facilitate patient discharges
  • Ensure timely and effective communication with other members of the MDT, the bed managers and partner agencies on all professional matters
  • To ensure Trust policies and guidelines are adhered to
  • To take part in clinical audits as required
  • To assist in audit and review complex patient discharge plans

Dimensions of job:


  • Day to day coordination of discharges at Heathlands Intermediate Care Unit
  • Partnership working with health and social care partners both within the acute hospital and community settings, particularly in relation to patient discharges
  • Data entry
  • Work as part of an MDT covering Mon-Fri
  • Use of EPIC for documentation and referrals

Key working relationships:


Internal:
clinical staff including nurses and therapists, Discharge Team, Community Services, Administrators, Unit Staff, doctors, pharmacists


External:
Adult Services, GPs, Transport, partner health organisations including but not limited to BHFT, provider agencies such as care homes and domiciliary care agencies


Purpose of job:


  • To facilitate the most appropriate discharge pathway for patients leaving hospital
  • To support cross organisation working and information sharing
  • To act as a single point of access to community services for patients who have been admitted and/or are needing support on discharge
  • To share information and signpost people to the most appropriate service
  • To facilitate appropriate information sharing in relation to supporting patient discharges
  • To complete referrals to external agencies
  • To facilitate patient case discussions to inform discharge
  • To use the EPIC system to track patients, pull reports, complete referrals and document interventions.
