Housing/Rough Sleeper Outreach Worker
Monday to Friday, 9:00-17:00 (11:00-19:00 on Tuesdays)


The role of a Rough Sleeper Outreach Worker is to manage a user caseload, devising, implementing, and reviewing ongoing housing and recovery plans and treatment, liaising closely with the full range of workers and skills within the service team, including family members and significant others as appropriate.

You will help set goals in recovery, mental health and help them achieve long term housing and work.


Duties include:


  • To provide a high quality service to clients presenting or in need, who have a history of substance misuse related problems and current housing concerns.
  • To provide prevention and early intervention support to reduce the number of clients presenting with treatment needs.
  • Manage a caseload of service users, helping them in their recovery to address their substance misuse issues and achieve a healthier lifestyle. This will include completing assessments and brief interventions with service users.
  • Provide relevant interventions based upon appropriate assessment or need and provide all relevant services in line with agreed targets and objectives.
  • Provide advice, advocacy and support to clients who have a history of problematic substance use.
  • Where appropriate to work in close partnership with other agencies to ensure clients access a full range of services.
  • Develop strategies to help reengage service users whom have dropped out of treatment due to physical barriers such as travel difficulties.
  • Work with local services to develop and maintain referral pathways.
  • Take a lead role in a specific service delivery area as agreed with the line manager and service manager.

Salary:
£18.00-£20.00 per hour

Expected hours: 35 per week
