Description

To work closely with Service Managers, Central Booking Office, Assistant Service Managers, Clinical Office Team Leaders, Appointments Centre, Clinical Office Staff and Outpatient teams within the division to ensure patients are actively managed through patient pathways.

To monitor patients pathways from referral, through diagnostics, treatment planning and treatment, highlighting any potential delays in the pathway and ensuring accurate data collection to support the process and report to clinical offices any discrepancies.

To ensure that appropriate requests for investigations are monitored following the clinicians instructions. If diagnostic delays occur or appear imminent liaise with the relevant service for action.


To liaise with members of individual specialties management teams and the Trust to ensure coordination of all elements of patient pathways.

This may include highlighting patient target dates etc. to ensure that investigations are carried out and results received in a timely manner.

To ensure all information relating to the patients pathway clearly identifies all relevant target dates to aid other users of the relevant systems, such as Cerner, evolve, LUNA etc in allocating resources appropriately.

To support the division in implementation of robust follow-up tracking, ensuring no patient is ever lost in the system.

Working with service managers monitor OP clinic capacity and support the clinical offices in backfilling vacant slots to ensure clinics are fully utilised.

To work with clinical offices in developing, implementing and monitoring changes to the patient pathway tracking/validating process and supporting improvement activities as required.

To maintain up to date knowledge of all standards, targets and processes relating to the patient pathway process and to be a point of reference for administrative teams across the Trust.


To provide, local training and support to other staff within the Trust regarding Referral to Treatment rules, processes and procedures.

To adhere to the Trusts Joint Access Policy and related national RTT Frameworks.
