Responsibilities:

Assess and assist individuals with their personal care and other activities of daily living.
Provide support to individuals in making decisions about their own lives.
Communicate with family members, service providers and others to coordinate the support services for individuals.
Maintain a safe and secure environment for the individuals in our care.
Document incidents for regulatory reasons.
Monitor health issues and arrange for care as required.
