Garden Lane Medical Centre Job Description Job Title Health Care Assistant Pay Band Band 2 or 3 depending on experience Directorate Garden Lane Medical Centre Line Manager Practice Nurses Nurse Manager Staff accountable to postholder None Job Summary The post holder will work under indirect supervision of the practice nursing team and nurse manager support the nursing and practice team in the delivery of tasks and duties to the practice population of Garden Lane Medical Centre


Principle Responsibilities:
Following initial assessment by a clinician, undertake delegated care for a variety of patients within the practice. Undertake delegated care packages within own level of competency for a variety of patient groups. Maintain accurate clinical records in accordance with practice policy.

To use standard procedures to ensure correct patient identification. To establish effective communication with the patients and carers to explain clinical procedures and allay fears. Communicate effectively with team members within the practice. Liaise effectively with partner services both voluntary and statutory from outside the practice.

Ensure nursing and practice equipment is clean and maintained. Order and maintain stock levels of equipment, vaccines and other supplies. Maintain a high standard of service in accordance with practice policy. Contribute to service development.

Work with service managers to ensure effective and efficient service delivery


Clinical:
Undertake, record and follow practice guidelines for a variety of tasks for which you have received appropriate training. Collect and prepare samples and specimens for transportation to the pathology laboratory for investigation.

Measure and record the following physiological measurements in routine presentations:

o Blood pressure o Pulse, rate and rhythm o Temperature o Height, weight and BMI o ECG o Doppler Undertake venepuncture in accordance with clinical guidelines.

Undertake a range of health checks, working to practice protocols and within competencies. Prepare and maintain the clinical environment and equipment, before, during and after use.

Assist members of the clinical team with patients undergoing minor surgical procedures Assist in raising awareness of health and well-being and how it can be promoted Undertake dressings and provide wound care in accordance with level of competence and utilizing local guidelines and procedures.

Have the skills to recognise life threatening circumstances and take appropriate action Have underpinning knowledge and practical skills of basic life support and recognition of anaphylaxis.

Provide support to patients and carers in the management of acute and long-term conditions. Act as a chaperone where needed and training has been completed. Re-stock clinical rooms with necessary supplies. Administer a range of vaccinations once training is completed, including Influenza, B12 and pneumonia vaccinations.

Fitting and removal of 24-hour blood pressure monitors. (this list is not inclusive of all job roles and responsibilities, but an example of the types of duties that will need to be performed.


Personal and People Development:
Maintain own clinical expertise by participating in training and development opportunities which meet own learning needs. Demonstrate a commitment to the training and education of learners within the clinical area as required.

Take responsibility for maintaining own record of personal development. Recognise the roles and responsibilities of individual members of the practice team. Be aware of the professional and legal limitations of own role. Awareness of the changing structures of health care provision both locally and nationally.


Communication:
Communicate effectively with patients and carers, recognising the need for alternative methods of communication. Offer support to nervous or distressed patients.

Record details of care provided in patients notes using the practice computer system Maintain patient confidentiality at all times in accordance with GLMC policy and Caldicott guidelines.

Advise managers on additional requirements, developments and needs of the service as appropriate.

Communicate effectively and clearly with professional colleagues and patients to ensure effective service delivery. Adapt personal communication skills to overcome barriers to communication. Utilize appropriate systems to communicate information to colleagues and patients.


Research and Audit:
Participate in research projects and audit of the service as required.


Managerial:
Organise own workload and work under indirect supervision.

Have knowledge of referral criteria and refer patients back to the appropriate clinician in cases of concern or should further advice/follow up be required.


Leadership:
