Description

Responsibilities:


  • Ensure the safety of the patient or client in the home setting.
  • Ensure that a daily plan of activities and care is completed for patient or client in the home setting.
  • Communicate with the physician, family, and other care givers regarding the patient or client's progress.
  • Work effectively with hospice staff and nursing personnel to coordinate care.
  • Interact with patient or client on a regular basis to know their needs and address any concerns.
  • Provide transportation to medical appointments and other functions as needed.
  • Maintain accurate records on a daily basis.

Salary:
£608.00-£649.00 per week
