About Reablement

Sometimes when people come out of hospital it doesn't always mean they are ready to go back to their previous life straight away.

Sometimes people might need a bit of extra support at home to stop them being admitted to hospital or needing long-term care.

It might be that temporary support at home is needed, until people get back to where they want to be.

This care is called intermediate care, or reablement.

The Role
The role is a unique blend of personal care and reablement work.

You will work with individuals in their homes to enable them to regain their independence and achieve their goals through independent living.

People are supported with a range of things, like cooking meals, managing their personal health and hygiene. They are supported to gain their confidence moving around their home safely, and in managing their daily activities.

You will work with other health and care professionals, such as GP's, physiotherapists, and community nurses, to collaboratively plan and implement personal care plans for each individual you support.

Not only will you make a positive difference to people's lives, but you will also have the pleasure of seeing that change first-hand.

Experience

Whilst previous experience of working in a health or care setting would be beneficial we will give you full training to meet the care needs of people we care for, which can range from motivational support to personal care.

Hours and Shift Patterns options available:
We offer a variety of weekend hours/shift patterns to suit:
Mornings 7 am pm

Evenings 3 pm pm

Split shifts also available (must be able to work Sat and Sun for this pattern).

Our minimum shift is 4 hours.

You can work alternate weekends.

Pay and Benefits
As this is a community based role, you will need your own transport. Mileage is paid at the rate of 45p per mile.

We have a small number of electric pool cars that could be available for short term use so if you are aged over 21 with a clean drivers licence then please get in touch.

We pay an enhanced rate pf time and a quarter for weekend working. The pay rated quoted includes the weekend enhancement.
