Description

To provide skilled nursing care working as part of the Primary Health Care Team to patients in a variety of community settings between the hours of 08:00 and 20:00, including weekends.

To actively monitor, maintain and develop the service provided. To take responsibility for managing the caseload and the team, with the support of the Team Leader. To deliver a proactive case managed approach for patients with long term care needs.
