About Us
It is an exciting time to join West Sussex County Council as we continue to introduce new ways of working in the hospitals to respond to the needs of the adult population.
The hospital Social Work Team, work within the Discharge to Assess model, responding within timescales to assess people both in the hospital and those that have recently been dischargedfrom hospital.
We assist them with arranging any care and support they may need in the longer term to maximise their independence.
The work is varied fast paced and there are opportunities to gain experience in working with a variety of professionals both in the Hospitals and Community Resources including the Voluntary sector.

£33,486 to £36,371 per annum

We're looking for a qualified social worker to join our hospital social work team
As a Social Worker, you will:
- Accountable for delivering a statutory social work service
- Have compassion and empathy to support individuals who need our services
- Work in partnership with colleagues to deliver a high standard of social work practice
- Work hard to support people to regain their skills and independence following their hospital admission wherever possible.
