Surrey Heath Community Providers Ltd (SHCPL, 'The Federation') / Surrey Heath Primary Care Network (PCN)


Base:
Surrey Heath Community Providers, Regus

The role will require working across the practices within the Federation / Surrey Heath Primary Care Network (PCN) footprint and the care home settings across the footprint, as well as a requirement to work with colleagues across the Integrated Care System (ICS)


Hours per week:37.5 hours per week (Full time). Part time hours considered


Salary:
Agenda for Change Band 4 Equivalent, depending on experience and qualifications (pro-rata for part time)


Annual Leave:27 days (based on full time hours, pro-rated for part time) plus Bank Holiday entitlement

Job Purpose:

The post holder is a Nursing Associate registered with the Nursing and Midwifery Council and we are able to support you to progress your career onto Registered Nurse in general Practice within the PCN.


Surrey Heath Community Providers offer a 365 day, 52 week service delivery and as part of that we offer a 7 days a week from 0800 hrs - 2000hrs service.

So you may be required to work evenings, weekends, and Bank Holidays as part of your normal working hours, as per the needs of the business.


Within this role you are required to act within the professional boundaries, supporting and working alongside a multi-disciplinary team within general practice and the Primary Care Network (PCN).


The post holder will contribute to providing nursing care to patients registered with the GP practices within the PCN, inclusive of undertake activities as outlined in the current network contract and associated direct enhanced services, like Personalised Care Support Planning and Anticipatory care assessing.


The post holder will focus on assuming a lead role in supporting the Trainee Nursing Associates and with a remit to support clinically as well as holding own clinical caseload to support the needs of vulnerable patients groups planning personalised care within the scope of Practice inclusive of those hard to engage with cohorts of patients, e.g.

Learning Disability [LD], Severe Mental Illness', Frail and Vulnerable.


The role will afford you engagement with colleagues in general practice, the PCN and wider community services to build extensive relationships and deliver, organise, engage to facilitate individualised holistic care providing support which may lead to intervention.

Working across the PCN with Multidisciplinary Teams inclusive of General Practice Nurses and GPs you will deliver collaborative care that focuses on high quality care adding impact and value to benefit patients, and the General Practice Workforce where appropriate.

This may include working collaboratively to organise and facilitate care appropriate to the individual needs of the patient.


Role and Responsibilities:
Communication and Relationship Skills

  • To engage in positive relationships with patients/carers/families and other staff based on respect and within appropriate boundaries.
  • To act in a professional capacity as outlined by the Nursing and Midwifery Council [NMC] Professional Code of Conduct for Nursing Associates and in accordance with the PCN values at all times.
  • To be able to set therapeutic limits with patients, carers or relatives.
  • To be able to communication with the patient about care planning risk assessment needs.
  • To communicate complex and sensitive information verbally and in writing, offering flexibility and adaption to ways of offering information and engagement.
  • To act as a role model to all professional groups, peers and positive represent SHCPL
  • To actively contribute to staff meetings acting as an ambassador for the profession, encouraging and inspiring your clinical peers to have a voice and as a Patient Advocate
  • To be able to confidently and accurately convene patient information in the role of an associate nurse for all Professional / MDT meetings.
  • To be able to articulate an understanding of SHCPL organisational structures along with championing the TNA/NA workforce role and responsibilities.
  • To formulate planned care with others and supervising / mentoring TNAs assigned under their care.
  • To communicate risk issues as they arise to ensure safe quality care.
Knowledge, Training and Experience

  • To be able to demonstrate evidenced based practice when undertaking care and delivering Health checks which may be working with patient cohorts who present with i.e., high blood pressure, diabetes, obesity, epilepsy, respiratory issues, constipation, lack of personal hygiene, including foot care.
