A new model of care has been developed in partnership with over 70 local clinicians and professionals across two major pathways: Care Closer to Home and Home First; to shift activity and resources from acute care to community services in Waltham Forest to improve the health and well-being of residents, to reduce inequalities in health care, and to support the delivery of a new Whipps Cross hospital.


In leading the implementation of the business case which sets out in detail the cost of delivering the new clinical model, the activity the new model of care will deliver, and the impact that the new community model will have on admissions and discharges to and from Whipps Cross hospital, ensuring that local people receive the right care at the right time and place.

The business case builds on the success achieved during the pandemic to ensure timely discharge from the hospital alongside establishing a proactive, anticipatory care approach to manage individuals with high and medium risk in the community.

The introduction of the new model represents a major transformation in how primary, community and secondary care work together.
