Job Summary:

We are seeking a compassionate and dedicated Disability Support Worker to join our team.

As a Disability Support Worker, you will provide assistance and support to individuals with disabilities, helping them to lead fulfilling and independent lives.

Your role will involve implementing care plans, providing personal care, and promoting the overall well-being of the individuals you support.

Duties:

Assist individuals with disabilities in their daily activities, including personal care, meal preparation, and medication administration.
Develop and implement care plans tailored to each individual's needs and goals.
Provide emotional support and companionship to individuals, fostering a positive and supportive environment.
Assist with mobility and transportation needs, ensuring individuals can access community resources and activities.
Collaborate with a multidisciplinary team to ensure coordinated care for individuals.
Maintain accurate documentation of services provided and any changes in the individual's condition or needs.
Utilize IT systems to record information, communicate with team members, and access relevant resources.

Qualifications:

Previous experience working with individuals with disabilities is preferred.
Excellent communication skills, both verbal and written.
Strong organizational skills and attention to detail.
Ability to work independently as well as part of a team.
Compassionate nature and a genuine desire to make a positive impact on the lives of others.
