Our client provides a Gender Identity Development Service is looking for a
Band 5 Senior Patient Referral Lead Administratorto assist with various projects ahead of our client's service closure.

Job Summary


To work closely with the management team to ensure and maintain a smooth intake, onward referral and discharge procedure for the service.

To support the clinical team(s) with their referral decision making processes.

To work as part of a fully integratedadmin team taking on tasks in collaboration with the other admin staff as directed by the Assistant Service Manager.


Duties

  • Being the shared first point of contact for potential referrers, staff, patients and their relatives and/or carers, providing information on the Service and the referral pathways, and prioritising queries as appropriate, particularly to take on more challengingqueries.
  • Keeping an overview of the logging of all referrals and maintaining referral data for use by the Service Manager, including referral status, waiting times, and decisions made.
  • To oversee the input of new referrals onto CareNotes
  • Ensuring that data for these referrals is accurate and complete and to check thoroughly for any previous referrals and being aware of issues in relation to name and gender change, informing Service Manager of any issues that arise.
  • Sending letters to referrers with decisions of intake as advised by the Intake team and liaising with the Service Manager around the use of template letters.
  • To assist the Assistant Service Manager with monitoring of referrals, until allocated.
  • Making initial informed decisions within scope of competence about the appropriateness of referrals and discussing these decisions with the Service Manager/lead clinicians.
  • Maintaining an overview of referrals to the Service, including referrals yet to be allocated and waiting times, and liaising with the Service Manager.
  • To oversee the process for presenting referrals to the Intake Teams for acceptance or refusal at the weekly intake meetings, and monitoring the progress of all referrals pending further information / decision.
  • To work closely with the Intake teams to support their administrative requirements.
  • To oversee the distribution of accepted referrals to Team members following discussion at the weekly team meetings.
  • To keep up to date with funding arrangements for the Service and appropriately arrange funding arrangements for referrals where required.
  • To work closely with the Named Patient Agreement Officer in the Contracts Team to ensure regular review of outstanding funding requests.
  • Ensuring all cases are allocated within the agreed waiting time, and that all staff are aware of this deadline.
  • Developing and maintaining relationships with professional referrers and outside agencies.
  • To oversee the preparation of electronic and paper files for all referred patients within two days of referral.
  • Ensuring the accuracy of the data, in particular the date of referral, referral received date, and the referral acceptance / rejection date.
  • Ensuring paper and electronic records are both accurate, complete, and commensurate.
  • To develop an awareness and knowledge of the specialist and clinical work undertaken in the Service.
  • To uphold the Trust's policy on confidentiality, promoting the careful and safe holding of patient information.
  • To work with the Service Manager in the provision of clinical data for the purposes of audit or reporting to commissioners.
  • To supervise and support Band 4 and Band 3 admin staff within the team assigned to intake and discharge related tasks, as required.
