Social Carer


Responsible to:
The Manager


Purpose:

To co-ordinate appropriate meaningful activities for the residents which encourage and value their abilities and enhance their quality of life.

Principal Duties - General

  • To carry out the duties and responsibilities of the post.
  • To become familiar with and adhere to Grange Hall Care Home's policies and procedures.
  • To adhere to health and safety legislation, adults at risk guidelines, National Care Standards etc.
  • To maintain confidentiality of all information relating to residents outwith Grange Hall.
  • To undertake mandatory and other appropriate training as stipulated by the Manager.
  • To participate, when possible, in staff meetings, residents' meetings and relatives meetings.
  • To participate in monthly supervision sessions with Manager.
  • To work as part of a team whose aim is to promote the general wellbeing of the residents as well as to achieve job satisfaction.
  • To carry out any other reasonable duties as requested by the Manager.
  • To treat residents with dignity and respect at all times.
  • To report to the Manager any issues concerning the health and care of the residents.
  • To work flexibly to meet the changing needs and demands of the service.
POST SPECIFIC Duties

  • To be responsible for continually developing a culture of the provision of activities.
  • To coordinate and plan the provision of meaningful activities in relation to the personal outcomes of each the resident eg minibus outings, craft sessions, gardening.
  • To complete an Interest Checklist with each resident shortly after admission.
  • To promote the engagement of individuals in physically, mentally and socially stimulating activities, encouraging each individual to achieve their potential and an appropriate degree of independence.
  • To assess each resident's residual abilities using the PAL Checklist and then compile an individual profile.
  • To deliver particular activity sessions for small groups or in 1:1 sessions.
  • To source and take responsibility for volunteer or specialist input to the activity programme when required eg communion organist, reflexologist.
  • To ensure that all equipment/materials/resources are kept in a clean and useable condition and to source new equipment/materials/resources when required.
  • To organise outings for the residents eg shopping trips, school visits etc.
  • To acquaint oneself immediately upon coming on duty with any relevant events or health issues relating to residents that may impact on their participation in certain events.
  • To facilitate the participation of residents in the running of Grange Hall Care Home eg meeting with the Chefs, residents' meetings
  • To escort and assist residents, when appropriate, in and around Grange Hall Care Home, eg to/from the Sensory Room, to/from the summerhouse etc
  • To facilitate social interaction between residents.
  • To set achievable goals for each resident and measure outcomes when appropriate.
Administrative Duties

  • To liaise with the Manager regarding the following month's activities.
  • Toliaise on a daily basis with the Social Carer/ Organiser and Supervisor of care staff.
  • To ensure that photos are downloaded from the camera onto the computer on a regular basis.
  • To ensure that the digital photo frame is kept uptodate and clean.
  • To remind staff of specific events when appropriate eg catering, housekeeping.
  • To participate in fundraising events eg Silent Auction, Christmas Fair.
  • To assess each resident's communication abilities.
  • Following Grange Hall guidelines, to record objectively and accurately, in the care plans, all interactions with/observations of residents.
  • To train staff in the provision of meaningful activity, and then involve them in activity provision, providing support and guidance as required.
  • To provide feedback to the Manager on each resident's participation in activities prior to their Social Work review and to assist in implementing any recommendations.
  • To encourage community involvement and develop community links eg Earlston Senior Citizens' Event.
  • To produce publicity material when required.
  • To keep accurate records eg evaluation of activities, feedback from events, minute of meetings.
  • To be responsible for maintaining a monthly budget, agreeing expenditure in advance with the Manager.

The post holder may be required to undertake other duties that may be required from time to time within the general scope of the post as requested by the Manager.

Any such duties would not change the general character of the post.

Duties and responsibilities outside of the general scope of this grade of post will be with the consent of the post holder.
