Clinical and Care Professional Leads support south east London-wide and borough programmes of work, therefore, there is an expectation that you currently work within south east London, and for borough based roles, within that borough.


The clinical care professional lead for CYP Long term Conditions and Integration is a key post within the South East London BCYP programme, enabling us to achieve the aims of the Forward View and the Core 20+5 requirements.

The postholder will support the priorities of the BCYP programme which include diabetes, epilepsy, and obesity as well as supporting the ongoing work around asthma and integration.


Interview date: 11th June 2024

  • Lead, inform and support SEL wide implementation of national and local
requirements and priorities in relation to Core20+5, LTC and integration,

  • Clinical input and leadership to the work of SEL Strategic Partnership Board
and Delivery board, and leading the CYP task and finish groups as required,

  • Convening and liaison with system partners ensuring a robust interface
between primary care, community, acute and mental health services

  • Working with Partners and other related programmes, to ensure that reducing
health inequalities is at the heart of our work with CYP and helping local
teams develop their work accordingly,

  • Leading the development of SEL wide responses to CYP LTC and integration
challenges
