We are looking to recruit an exceptional individual to join our Lung Cancer Nursing Team to take on the role of Cancer Care Co-ordinator for lung cancer.

You will work as a member of the team providing patient coordination and care, for Lung Cancer patients.

You will work with the Clinical Nurse Specialists (CNS) to provide coordination of high quality patient care through on-going telephonic/ face to face assessment and proactive identification of needs utilising basic clinical knowledge, approved tools and procedures.

support the Lung Cancer CNS team (5 nurses) in delivering a high standard support to patients with cancer. This will involve being in daily contact with patients suffering from a range of cancers based in the lung.

The post holder will be expected to support and signpost patients over the telephone and progress their care pathway in liaison with other colleagues and departments in the hospital.

The post holder will also be expected to be involved with audit and Peer Review processes.

In conjunction with the Clinical Nurse Specialists (CNS) provide coordination of high quality patient care through ongoing telephonic/ face to face assessment and proactive identification of needs utilizing basic clinical knowledge, approved tools and procedures.


Collaborate and communicate with the CNS and wider Health Care Team to ensure the provision of appropriate services to maximise the health and quality of life of the patient, improving the quality and efficiency of health care delivery.

Single point of contact for patient, carers and the health care team. Triaging enquires to the appropriate department within the hospital or community setting. Actively participate in the provision of routine information and advice, for patients and carers over the telephone, liaising with the CNS as appropriate

Document and monitor all aspects of patient care coordination and service delivery, supporting data collection for audit
